  To request a sacramental certificate for sacraments received at St. Christopher's Catholic Church, please complete the online form below. A member of our parish staff will follow up with you. Please allow one week for processing.
  
  There is a $10 processing fee, which can be paid in person at the Parish Office with Cash, Check, or Credit Card at pickup or on the telephone with the Parish Secretary for mailed documents.
  
  Baptismal Certificate with notations:
  
  A Baptismal Certificate (dated within 6 months of your wedding) is needed before you can be married in the Catholic Church. The certificate must be signed, dated, and imprinted with the Church's seal.
  
  For accurate processing, please indicate on the form which sacraments you have received at St. Christopher's.
  
  In accordance with parish policy, certificates may only be picked up in person at the parish office or mailed directly to the Catholic parish/church nearest to you. If you need assistance or have any questions, please contact the parish secretary at (702) 642-1154 or email [email protected].
